Online Applications

Individuals can submit an online examination and endorsement application through the Ohio e-License Center as an alternative to downloading and mailing the paper application. A link to the Online Initial License Application site is located on the Information menu on the left-hand column of this page. After applying online, applicants are required to submit the Certification of Applicant for Online Applications document, in addition to any other supporting materials required as part of the application.

• Examination Application - This application is for individuals seeking approval to sit for the National Physical Therapy Examination (NPTE).

• Endorsement Application - This application is for individuals who have passed the National Physical Therapy Examination (NPTE) and who hold a current, valid license to practice physical therapy in another U.S. state or territory.

• Reinstatement Application - This application is for individuals who previously held a license to practice physical therapy in Ohio at any time.

• Ohio Jurisprudence Examination Candidate Handbook - All applicants for licensure (examination, endorsement, and reinstatement) are required to take the online Ohio Physical Therapy Jurisprudence Examination as part of the application process.
