The Ohio Revised Code authorizes the Board to investigate compliance with the laws and rules governing the practice and to investigate alleged grounds for discipline of a license or refusal to issue a license. The following documents include an explanation of the complaint and investigation process and the actual form needed to file a complaint against a licensee.
